Medical

Medical coverage is still provided through Blue Cross Blue Shield of AZ.

To find out more about the Blue Cross network: azblue.com / find care / browse the network / select 2025 as coverage year and employer provided under type of coverage / select medical under type of provider and then Statewide / National PPO.

Vision

Vision coverage is still provided through Superior Vision.

We are pleased that premiums remain unchanged.

Waiver Payments

By submitting proof of alternative medical and/or dental coverage, you may opt to waive Leona's coverage and receive a waiver payment, which will be included in each of your paychecks.

FSA Accounts

FSA accounts are still managed by WEX. Remember, re-enrollment is required to participate annually.

FSA coverage period: July 1 through June 30.

  • Medical FSA - contribution limits $3,300 (for 2025)
  • Medical FSA rollover: Up to $660 can rollover into the next plan year.
  • Dependent Care FSA - contribution limits $5,000 (or $2,500 if married and filing separately)
  • Dependent Care FSA rollover: Unused funds do not rollover—use it or lose it.

Voluntary Term Life

Voluntary term life insurance for employees, children, and spouses is still provided through The Hartford.

You can enroll in new coverage or increase coverage. Any new enrollment and/or increase in coverage will require the completion of a medical questionnaire.
