Incident Reporting Forms

Leona Schools Injury Report (Student and Staff)

Purpose: Please complete this form for all injuries that occur on campus or during school-sponsored activities, regardless of severity.

When to Use:

  • Any student injury requiring first aid or medical attention
  • All staff injuries, including minor incidents
  • Injuries during field trips, sports, or extracurricular activities
  • Any incident that may require follow-up medical care

Important: Reports must be submitted within 24 hours of the incident. Include detailed information about what happened, where it occurred, who witnessed it, and what first aid or medical treatment was provided.

Leona Schools Accident Report

Purpose: Use this form to report accidents involving vehicles, equipment, or other incidents not covered by the injury report form.

When to Use:

  • Vehicle accidents involving school vehicles or occurring during school business
  • Equipment malfunctions or failures
  • Near-miss incidents that could have caused injury
  • Environmental hazards (spills, structural damage, etc.)

Important: All accidents must be reported immediately. For vehicle accidents, also contact local authorities and your insurance provider. Take photos of the scene if safe to do so.

Leona Schools Property Damage / Vandalism Report

Purpose: Report any damage to school property, whether accidental, due to vandalism, or caused by natural events.

When to Use:

  • Vandalism or graffiti
  • Broken windows, doors, or fixtures
  • Damaged technology or equipment
  • Theft or missing property
  • Weather-related damage
  • Any other property loss or damage

Important: For vandalism or theft, also file a police report and provide the case number on this form. Document damage with photos if possible. Do not attempt to repair or clean up until authorized by Risk Management.

Important Safety Reminders

Report All Incidents Immediately

All workplace incidents, injuries, and near-misses must be reported within 24 hours, even if they seem minor. Early reporting ensures proper care and documentation.

Emergency Procedures

Familiarize yourself with emergency exits, assembly points, and evacuation procedures at your location. In case of emergency, dial 911 first, then notify school administration.

Safety is Everyone's Responsibility

If you see a potential hazard, report it immediately to your supervisor or the Risk Management team. Don't assume someone else will handle it.

Document Everything

When reporting an incident, provide as much detail as possible. Include dates, times, locations, witnesses, and any actions taken. Photos can be extremely helpful for documentation.
